The Paul Revere House accepts reservation requests by phone and fax. (We do not accept program requests by email.) No matter which method you use, please complete this form first. You may fax it to us or use it as a reference while talking with us on the phone.

You do not have a reservation until we have confirmed the arrangements with you by telephone and you have received a written confirmation.

	Midnight Ride Storytelling Program, Gr. K-3 Up to 22 students, two programs may be scheduled concurrently 90 minutes; one hour if presented at your school
